September 16, 2020 Opinion or Order Filing 4 ORDER: Accordingly, it is ORDERED as follows:(1) Mayes's initial submission, which was docketed as a civil rights complaint for administrative purposes [DE #1 ], is DISMISSED without prejudice to his right to file a new, proper civil rights action. (2) This specific action is STRICKEN from the Court's docket. (3) However, the Clerk's Office shall send Mayes the following blank forms: (a) a Court-approved Civil Rights Complaint form [E.D. Ky. 520 Form]; (b) an Application to Proceed in District Court Without Prepaying Fees or Costs [AO 240 Form]; and (c) a Certificate of Inmate Account Form [E.D. Ky. 523 Form]. (4) If Mayes wishes to initiate a civil rights case, he may do so by completing and filing a new action in accordance with the instructions set forth in this Order. Signed by Judge Joseph M.
