The Clerk's Office is directed to send Byrd the following blank forms: a. a Court-approved form petition for a writ of habeas corpus pursuant to 28 U.S.C. 2241 [E.D. Ky. 521 Form]; b. an Application to Proceed in District Court Without Prepaying Fees or Costs [AO 240 Form]; andc. a Certificate of Inmate Account Form [E.D. Ky. 523 Form]. 5.
