ORDER:1. Defendants SHALL FILE Response to Robinson's motions for injunctive relief #11 , #13 within fourteen (14) days. ORDER:1. Robinson's claims against Dr. Stella are DISMISSED with prejudice. 2. Robinson's Emergency Motion for Temporary Restraining Order #5 is DENIED. 3. Deputy Clerk shall prepare a "Service Packet" for Laurel County, Kentucky, Jailer Jamie Mosley, Medical Administrator Jamie Hart, and APRN Chastity. Each Service Packet shall include: a.) completed summons form, b.) the Complaint #1 , c.) this Order, d.) completed USM Form 285. 4. Deputy Clerk shall deliver Service Packets to USMS in Lexington, Kentucky and note the date of delivery in the docket. 5. USMS shall make arrangements with appropriate officials at the Laurel County Detention Center to personally serve Jailer Jamie Mosley, Medical Administrator Jamie Hart, and APRN Chastity with Service Packets at: Laurel County Detention Center, 440 Hammock Road, London, KY 40744. 6. ORDER:1. Court GRANTS Robinson's Motion for Leave to Proceed in forma pauperis #3 . 2. Because Robinson has been granted pauper status in this proceeding, the $52.00 administrative fee is waived. 3. Clerk shall open an account in Robinson's name for receipt of filing fee. Clerk shall complete a Notice of Payment Form (Form EDKY 525) and send Notice and a copy of this Order to warden of institution in which Robinson is confined. 4. Within twenty-eight (28) days, Robinson's custodian shall deduct $17.42 from Robinson's inmate account and remit that sum to Clerk as an initial partial filing fee. 5. For each month thereafter, if amount in Robinson's inmate account exceeds $10.00, custodian shall deduct an from account an amount equal to 20% of his income for the preceding month and remit that sum to Clerk.
