Filing 9 ORDER: Ellis Smith is an inmate at the Roederer Correctional Complex in LaGrange, Kentucky. Proceeding without a lawyer, Smith recently filed a civil rights complaint [R. #1 ], as well as a motion for leave to proceed in forma pauperis [Rs. #3 , #4 ]. Accordingly, it is ORDERED as follows: 1. Smith's motion for leave to proceed in forma pauperis [R. 3] is GRANTED. Payment of the $52.00 administrative fee is WAIVED. Pursuant to 28 U.S.C. 1915(b), Smith must pay the $350.00 filing fee in the manner set forth below. 2. Within 30 days from the entry of this Order, Smith must pay $26.44 as an initial partial filing fee. 3. The Clerk shall open an account in Smiths name for receipt of the filing fee. The Clerk shall complete a Notice of Payment Form [E.D. Ky. 525 Form] with:(a) Smith's name; (b) his inmate registration number; and (c) this civil case number. The Clerk shall then send a copy of this Order and the Notice of Payment Form to the Warden of the institution where Smith is confined. 4. Smith's custodian shall send the Clerk of this Court a payment each month from his inmate trust fund account. 5. This matter now stands submitted for initial screening. Signed by Judge William O.
