ORDER: 1. Jones's motion to proceed in forma pauperis [Record No. #5 ] is GRANTED. 2. Because Jones has been granted pauper status, the Court WAIVES the $52.00 administrative fee that would otherwise apply. 3. The Clerk of Court is directed to open an account in Jones's name for receipt of the filing fee. The Clerk is further directed to complete a Notice of Payment Form (Form EDKY 525) and send the Notice and a copy of this Order to the warden of the institution in which Jones is confined. 4. Because Jones lacks the funds to pay the fee, collection of the initial partial filing fee is DEFERRED. 5. For each month hereafter, Jones's custodian is directed to send to the Clerk of the Court a payment from his inmate trust fund account, but only if the account balance is more than $10.00. The payment should equal 20% of the amount deposited into Jones's account during the preceding month. The custodian is directed to continue the monthly payments until the entire $350.00 filing fee is paid. 6. A Deputy Clerk is directed to prepare "Service Packets" for service upon: (a)-(d). Each Service Packet shall consist of: (a)-(d). 7. The Deputy Clerk is directed to send the Service Packets to the USMS in Lexington, KY and note the date of delivery in the docket. 8. The USMS is directed to serve the United States of America by sending a Service Packet by certified or registered mail to: (a) and (b). 9. USMS is directed to serve the BOP by sending a Service Packet by certified or registered mail to: (a)-(c). 10. USMS is directed to personally serve Lt. Hurley and Officer J.
