ORDER: 1. Court GRANTS Motion for Leave to Proceed IFP, D.E. #3 , and waives the applicable filing and admin fees. 2. Clerk' Office DIRECTED to send Elder a blank "Complaint for Violation of Civil Rights (Non-Prisoner)" Form, Pro Se 15 Form. 3. If Elder wants to proceed with this matter, then he must Complete the Pro Se 15 Form in full, sign it, and file it with this Court within 30 days from the entry of this Order. If Elder does not complete, and, and file form within the time allotted, the Court will dismiss this action without prejudice. 4. Finally, Elder must inform the Clerk's Office, in writing, of any change in his address. If fail to do so, Court will dismiss his case. Signed by Judge Robert E.
