July 24, 2023 Opinion or Order Filing 4 ORDER: 1. The Clerk's Office is directed to send Young the following blank forms: a. an Application to Proceed in District Court Without Prepaying Fees or Costs [AO 240 Form]; and b. a Certificate of Inmate Account Form [E.D. Ky. 523 Form]. 2. Within 30 days from the entry of this Order, Young must (a) pay the $5.00 filing fee as required by 28 U.S.C. 1914, (b) move for leave to proceed in forma pauperis by completing and filing the AO 240 and E.D. Ky. 523 Forms, or (c) submit a copy of a BP-199 Form that he filed with prison officials requesting that the prison withdraw funds from his inmate account to pay the $5.00 filing fee. 3. The Clerk's Office shall send a copy of Youngs petition [R. 1] and this Order by certified mail to both (a) the Warden at the United States Penitentiary McCreary in Pine Knot, Kentucky; and (b) the United States Attorney's Office for the EasternDistrict of Kentucky. 4. By no later than September 19, 2023, the United States Attorney's Office for the Eastern District of Kentucky must file a response to Young's petition on behalf of the Respondent in this case. 5. Finally, Young must promptly inform the Court, in writing, of any change in his mailing address. If Young fails to do so, the Court may dismiss his case. Signed by Judge Karen K.
