ORDER: 1. Bowling's motion for leave to proceed in forma pauperis [R. #8 ] is GRANTED. The Court WAIVES payment of the filing and administrative fees. 2. A Deputy Clerk shall prepare a "Service Packet" for defendant Cabinet for Health and Family Services. Each Service Packet shall include, in Portable Document Format ("PDF"), the following: a. a completed summons form; b. the amended complaint [R. #7 ]; and c. this Order. 3. The Deputy Clerk shall attach the Service Packet as an attachment to an e-mail directed to: ServetheCommonwealth@ky.gov. The subject line of the email shall state "Service of civil complaint upon Cabinet for Health and Family Services." The Clerk shall note the date of delivery in the docket text.
