ORDER: 1. Hood's motion to proceed in forma pauperis #3 is GRANTED. 2. Within 28 days, Hood must pay $32.00 to Clerk as an initial partial filing fee. 3. Clerk shall open an account in Hood's name for receipt of filing fee. Clerk shall complete a Notice of Payment Form with (a) Hood's name, (b) his inmate registration number, and (c) this case number. Clerk shall serve a copy of this Order and Notice of Payment Form upon Jailer/Warden of institution in which Hood is currently confined; 4. After initial partial filing fee is paid, each month Hood's custodian shall send Clerk a payment in an amount equal to 20% of his income for preceding month out of his inmate trust fund account. The custodian shall continue such monthly payments until entire $350.00 filing fee is paid. 5. Hood must immediately advise Clerk's Office of any change in his current mailing address. Failure to do so may result in dismissal of this case; and 6. This matter stands submitted for initial screening. Signed by Judge Gregory F. Van Tatenhove on 12/17/2018.
