Filing 4 ORDER: 1. Clark's complaint R. #1 is DISMISSED WITHOUT PREJUDICE;2. The Clerk SHALL SEND Clark a civil rights complaint form [EDKYForm 520], a Federal Habeas Form [EDKY Form 521], an Affidavit of Assets/In Forma Pauperis Application [AO-240 Form], and a Certificate of Inmate Account Form [EDKY Form 523]; 3. Clark may file a civil rights complaint regarding his conditions of confinement claims. However he must use the Court-approved complaint form to do so; 4. If Clark does file a civil rights complaint, he must also either pay the $400.00 in administrative and filing fees in full at the time he files the complaint, or ask to pay the filing fee in installments over time by completing the following steps: See (a) - (c). 5. If Clark wishes to file a petition for a writ of habeas corpus pursuant to 28:2241, he should complete the Federal Habeas Form [EDKY 521]. Clark must also either pay the $5.00 filing fee or seek leave to proceed as a pauper bycompleting the following steps: See (a) - (c). 6.
