ORDER: (1) Clerk shall send Wright a Petition for Writ of Habeas Corpus [EDKY Form 521], an Affidavit of Assets/In Forma Pauperis Application [Form AO-240] and a Certificate of Inmate Account [EDKY Form 523]; (2) Within 30 days, Wright must complete & file the habeas petition form in its entirety; (3) Within 30 days, Wright must also either pay the $5.00 filing fee or (a) have the Certificate of Inmate Account [EDKY Form 523] certified by prison staff, (b) complete the Affidavit of Assets/In Forma Pauperis Application [Form AO-240], and (c) file both of them with the Court; and (4) The Court will deny the petition without prejudice if Wright does not take each of these required steps within 30 days.
