ORDER: 1. Mr. Bucklers motion to proceed in forma pauperis (R. #3 ) is GRANTED. Section 1915(b)(1) requires a prisoner-plaintiff to pay $350 filing fee for civil action; a. Buckler indicates that he lacks sufficient income, and payment of such fee is therefore DEFERRED; b. Clerk open account for receipt of filing fee. Clerk complete Notice of Payment Form (Form EDKY 525) with (a) name, (b) inmate registration number, and (c) case number. Clerk serve copy upon the Jailer/Warden and Department of Corrections; c. custodian send Clerk payment in amount equal to 20%, only if amount exceeds $10 until pd in full. 2. Clerk mark assigned case number on form Civil Rights Complaint [EDKY Form 520] and send to Buckler; 3. Within 30 days, Buckler must file an amended complaint using form provided. Buckler may also include attachment, not exceeding 6 pgs, setting forth addtl facts and claims; and 4. Buckler must take these steps w/in 30 days, or case will be dismissed without prejudice for failure to prosecute and failure to comply with a Court Order. Signed by Judge Gregory F.
