November 2, 2022 Opinion or Order Filing 10 ORDER: 1. Taylor's motion for leave to proceed in forma pauperis (R. #8 ) is GRANTED, and payment of $52 administrative fee is WAIVED. 2. Pursuant to 28:1915(b), Taylor SHALL pay $350 filing fee. 3. Within 30 days, Taylor must pay $58.70 to Clerk as an initial partial filing fee. See (R. #9 , p. 2). 4. Clerk SHALL open account for receipt of filing fee. Clerk complete a Notice of Payment Form [E.D. Ky. 525 Form] with: (a) name, (b) inmate registration number, and (c) case number. Clerk send copy of Order and Notice Form to Warden of institution Taylor currently confined. 5. Custodian SHALL send Clerk a payment each month in amount equal to 20% of Taylor's income from preceding month, if account exceeds $10. Custodian continue monthly payments until entire filing fee paid. 6. This matter now stands submitted for initial screening pursuant to 28:1915A and 1915(e)(2). Ky. 523 Form. 4. Within 30 days, Taylor must either (a) pay $402 in filing and administrative fees up front and in full or (b) properly ask to pay filing fee in installments over time by completing and filing both the AO 240 & EDKy 523 Forms. If Taylor chooses to file AO 240 & EDKy 523 Forms, the latter form must be certified (signed) by a prison official. 5.
