ORDER: 1. Warick's motion for leave to proceed in forma pauperis, DE #2 , is DENIED without prejudice. 2. Clerk directed to send Warick following blank forms: a. an AO 240 Application to Proceed in District Court Without Prepaying Fees or Costs; and b. an E.D. Ky. 523 Certificate of Inmate Account Form. 3. Within 30 days, Warick must either pay $405.00 or properly ask to pay filing fee in installments over time by completing and filing both the AO 240 and E.D. Ky. 523 Forms. If Warick fails to take these steps within time allotted, Court will dismiss case without prejudice. 4. Finally, Warick must inform Clerks Office, in writing, of any change in his address.
