Filing 5 MEMORANDUM AND ORDER: IT IS HEREBY ORDERED that plaintiff is granted leave to proceed in forma pauperis. IT IS FURTHER ORDERED that the plaintiff shall pay an initial filing fee of $3.10 within thirty (30) days of the date of this Order. Plaint iff is instructed to make his remittance payable to "Clerk, United States District Court," and to include upon it: (1) his name; (2) his prison registration number; (3) the case number; and (4) that the remittance is for an original proceed ing. IT IS FURTHER ORDERED that the Clerk shall mail to plaintiff a Prisoner Civil Rights Complaint. IT IS FURTHER ORDERED that petitioner shall file an amended complaint on the provided form no later than thirty (30) days from the date of this Order . IT IS FURTHER ORDERED that if plaintiff fails to comply with this Order, this action will be dismissed. Initial Partial Filing Fee due by 3/14/2014. Signed by Magistrate Judge Lewis M.
