Plaintiff is instructed to make his remittance payable to "Clerk, United States District Court," and to include upon it: (1) his name; (2) his prison registration number; (3) the case number; and (4) the statement that the remittance is for an original proceeding. IT IS FURTHER ORDERED that the Clerk of Court is directed to issue process or cause process to issue on defendants Jason Lewis, Benjamin Crass, and Cole Hansen in their individual capacities as to plaintiff's claims of failure to protect. IT IS FURTHER ORDERED that the Clerk of Court is directed to issue process or cause process to issue on defendant Megan Unknown in her individual capacity as to plaintiff's claim of deliberate indifference to medical needs, pursuant to the waiver agreement the Court maintains with Corizon.
