Filing 5 OPINION, MEMORANDUM AND ORDER: IT IS HEREBY ORDERED that plaintiff's motion to proceed in forma pauperis [ECF No. #2 ] is GRANTED. IT IS FURTHER ORDERED that plaintiff shall file an amended complaint on the Court-provided form within twenty-one (21) days of the date of this Order. Plaintiff is advised that his amended complaint will take the place of his original complaint and will be the only pleading that this Court will review. Plaintiff should not include his child custody claims in his amended complaint. Additionally, claims relating to his arrest and imprisonment are likely Heck- barred and unable to be reviewed by this Court. IT IS FURTHER ORDERED that plaintiff's motion for appointment of counsel [ECF No. #3 ] is DENIED AT THIS TIME. IT IS FINALLY ORDERED that if plaintiff fails to comply with this Order, the Court will dismiss this action without prejudice and without further notice.
