July 9, 2014	Filing 7 MEMORANDUM AND ORDER: HEREBY ORDERED that plaintiff's motion for leave to supplement his complaint [Doc. #6] is DENIED. FURTHER ORDERED that plaintiff shall submit a certified copy of his prison account statement for the six- month period immed iately preceding the filing of his complaint within thirty (30) days of the date of this Order. FURTHER ORDERED that if plaintiff fails to submit a certified copy of his prison account statement for the six-month period immediately preceding the fi ling of his complaint, within the time specified, the case will be dismissed, without prejudice. FURTHER ORDERED that the Clerk shall mail plaintiff a copy of the Court's Prisoner Civil Rights Complaint form. FURTHER ORDERED that plaintiff shall have thirty (30) days to file an amended complaint, on the court-form that will entirely supercede his original complaint. Plaintiff's failure to comply with this Order will result in a dismissal of this action, without prejudice. ( Response to Court due by 8/9/2014.) cc: forms mailed.
