November 9, 2015 Opinion or Order Filing 9 MEMORANDUM AND ORDER. (See Full Order.) IT IS HEREBY ORDERED that the Clerk of Court shall provide plaintiff with a financial affidavit form that plaintiff must complete and send back to this Court. IT IS FURTHER ORDERED that the Clerk of Court sh all provide to plaintiff, along with a copy of this Order, a blank complaint form for the filing of an employment discrimination complaint. IT IS FURTHER ORDERED that plaintiff shall file an amended complaint, in accordance with the instructions set forth above, no later than thirty (30) days from the date of this Order. IT IS FURTHER ORDERED that plaintiff shall file a completed financial affidavit no later than thirty days from the date of this Memorandum and Order. Plaintiff's failure t o file a completed financial affidavit within thirty days of the date of this Order will result in a dismissal of this case without prejudice. IT IS FURTHER ORDERED that if plaintiff fails to timely file an amended complaint or fails to comply with the instructions set forth above relating to the filing of the amended complaint, the Court shall dismiss this action without prejudice.
