Filing 3 MEMORANDUM AND ORDER - IT IS HEREBY ORDERED that plaintiffs motion for preliminary injunction and temporary restraining order is DENIED without prejudice. IT IS FURTHER ORDERED that the Clerk of Court shall send to plaintiff a copy of the Courts Prisoner Civil Rights Complaint form. IT IS FURTHER ORDERED that the Clerk of Court shall send to plaintiff a copy of the Courts Motion to Proceed In Forma Pauperis Prisoner Cases form. IT IS FURTHER ORDERED that plaintiff shall file an amended complaint on the Court-provided form within thirty (30) days of the date of this order in accordance with the instructions set forth above. IT IS FURTHER ORDERED that plaintiff shall either pay the $400 filing fee or submit a motion to proceed in forma pauperis within thirty (30) days of the date of this order. IT IS FURTHER ORDERED that if plaintiff fails to comply fully and timely with this order, the Court will dismiss this action without prejudice. If the case is dismissed for non-compliance with this order, the dismissal will not constitute a strike under 28 U.S.C. 1915(g). IT IS FURTHER ORDERED that upon the filing of the amended complaint, the Court will review the amended complaint pursuant to 28 U.S.C. 1915. Amended/Supplemental Pleadings due by 4/1/2019..
