IT IS FURTHER ORDERED that the Clerk of Court shall mail to plaintiff one blank Civil Complaint form. Plaintiff may request additional forms as needed. IT IS FURTHER ORDERED that, within thirty (30) days of the date of this Order, plaintiff must file a third amended complaint in accordance with the instructions herein and the instructions in the Court's January 5, 2023, Memorandum and Order. MEMORANDUM AND ORDER: IT IS HEREBY ORDERED that the Clerk of Court is directed to mail plaintiff a copy of the Court's 'Application to Proceed in District Court without Prepaying Fees or Costs' form. IT IS FURTHER ORDERED that the Clerk of Court shall mail to plaintiff two blank Civil Complaint forms. Plaintiff may request additional forms as needed.
