Text of Order: The Application to Proceed Without Prepayment of Fees or Costs (doc. no. 2) is denied without prejudice. The plaintiff failed to provide the required information in the form, thus depriving the court of the ability to determine whether the payment of the fee will deprive the plaintiff and any dependents of the "necessities of life." Adkins v. E.I. Dupont Nemours Co., 335 U.S. 331, 339 (1948). The plaintiff may resubmit the request to waive the filing fee on a form provided by the clerk's office.
