The plaintiff is directed to fill out those forms out for each defendant and return them to the court within thirty days of this Order. The clerk's office shall issue the summonses upon receipt and provide them to the United States Marshals Service (USMS), along with a copy of the complaint (Doc. No. 1) and this Order.
