Such service shall be executed by regular U.S. mail and accompanied by a notation reading, IN CONNECTION WI TH STATE OF NEW JERSEY v. VINCENT MAHAM, INDICTMENT No. 3541-12-13. ORDERED that the Clerk shall serve this Memorandum Opinion and Order upon Plaintiff by regular U.S. mail and shall enclose in said mailing a blank civil complaint form and a blank in forma pauperis application for incarcerated individuals seeking to commence a civil matter.
