Plaintiff's claims against DEPARTMENT OF CORRECTIONS, BUREAU OF PRISONS, STATE OF NEW JERSEY, and NORTHERN STATE PRISON are DISMISSED with prejudice. Plaintiff's claims against NORTHERN STATE PRISON MEDICAL DEPARTMENT is DISMISSED without prejudice. Plaintiff's excessive force claim against SGT. VLEZ may PROCEED. The Clerk shall provide Plaintiff with a copy of the USM-285 form for SGT. VLEZ. Upon Plaintiff's sending of the completed form to the Clerk of the Court, the Clerk shall issue Summons, and the United States Marshal shall serve a copy of the Complaint, Summons, and this Order upon SGT.
