Question 3 is incomplete, in that, although box 3(d) is checked yes, it fails to thereafter provide information as to the source of money, amount received, and amount expected to be received in the future. As a result, IFP application #3 is DENIED. If Plaintiff wishes to proceed in this action IFP, he shall by 4/15/2024 submit a full and completed IFP application with all questions answered completely.
