Plaintiff shall make additional monthly payments of $25.00 every month thereafter until the $402.00 filing fee is paid or this action will be dismissed without further notice to him. FURTHER that proceedings in this this case are STAYED for 30 days, to allow Plaintiff the opportunity to submit his claims on the proper forms. Failure to submit the proper forms will result in the case being dismissed without prejudice. The Clerk is directed to send to Plaintiff the Pro Se Complaint for Civil Case forms. If Plaintiff submits his claims on the proper forms, the case should be referred back to the Court for further review to determine whether the case should proceed. FAILURE TO COMPLY WITH THIS ORDER IN A TIMELY MANNER WILL RESULT IN DISMISSAL OF THIS ACTION WITHOUT FURTHER NOTICE TO PLAINTIFF. (Sheets, Jamie)
