ORDER GRANTING LEAVE TO PROCEED IN FORMA PAUPERIS signed by MAG/JUDGE JOE L. WEBSTER on 9/8/2022; that the plaintiff in the above-entitled proceeding be, and is hereby, permitted to commence said proceeding in this Court without prepayment of fees or costs, or giving security therefor. [28 U.S.C. 1915(a)] The plaintiff is responsible for preparing and delivering to the Clerk, the correct summons for service on each defendant, including the correct address and the name and title of the individual to be served on behalf of a corporation, association, infant, incompetent or government agency. Failure to prepare and deliver said summons within 15 days from the filing of this order shall result in this case being dismissed without further notice. The U.S. Marshal shall serve the summons and complaint upon each defendant. IT IS FURTHER ORDERED that the Clerk shall terminate the plaintiffs original application to proceed in forma pauperis (Docket Entry #1 ). (Carter, Alexus)

| TEXT ORDER: This matter has been referred to the undersigned for review of #1 Plaintiff's application to proceed in forma pauperis ("IFP"). It appears that Plaintiff filed a document indicating the funds she receives monthly from a governmental source. However, to fully assess whether or not Plaintiff qualifies for IPF status, the Court will ORDER Plaintiff to complete an application to proceed IFP. IT IS THEREFORE ORDERED that the Clerk shall send Plaintiff a copy of the Application to Proceed IFP (long form) along with this Order. IT IS FURTHER ORDERED that Plaintiff shall have up to and including Friday, September 16, 2022 to fully complete, sign, and submit said form to the Court.
