ORDER that the Clerk is directed to mail 2 blank summons forms to Pltf to fill out and identify Deft McFadden and Deft John Doe in the summonses for service of process, and then return the summonses to the Court. Pltf is required to provide the necessary information for the U.S. Marshal to effectuate service. Once the Court receives the summonses from Pltf, the Clerk shall then direct the U.S. Marshal to effectuate service upon Defts. FURTHER ORDERED that Defts Mecklenburg County Jail, Mecklenburg County Sheriff's Office, Mecklenburg County Chaplain's Office, and Pltf's Fourteenth Amendment equal protection, RLUIPA, and official capacity claims are DISMISSED. FURTHER ORDERED that Pltf's #4 Motion for Clarification is DENIED without prejudice in accordance with the terms of this Order.
