Filing 4 Order: Pro se plaintiff Marquita Phillips filed a civil action in this Court. She did not pay the filing fee for this action. Rather, she submitted an Application to Proceed without Prepaying Costs or Fees (Doc. No. #2 ). The short form she used, however, is an outdated form that does not provide a complete and accurate picture of her financial situation. The Court notes that Plaintiff has been previously warned about the deficiency of this outdated Application in a case previously filed in this district. See Phillips v. Santander Bank, No. 1:22-cv-01354 (N.D. Ohio filed Aug. 1, 2022) (Ruiz, J.). Plaintiff is therefore ordered to either pay the full fee of $ 405.00 or to complete and submit the accompanying Application to Proceed In Forma Pauperis within 14 days of the date of this Order.
