Order Plaintiff seeks to pursue a civil rights action in this Court. He, however, did not submit the filing of $ 405.00 with his Complaint. Instead, he created a document he titles as a Motion to Proceed In Forma Pauperis. (Doc. No. #2 ). That document is not one that is approved by the Court, nor does it contain the required information.Plaintiff is incarcerated in FCI Allenwood. As a prisoner, he must complete a Prisoner Financial Application and submit a certified copy of his prison trust account statement for the six months immediately preceding the filing of his Complaint. He did not complete the Prisoner Financial Application and did not attach a certified copy of his prison trust account statement. Without these documents, his Application is deficient.Plaintiff is ordered, within 30 days of the date of this order, to either to pay the full fee of $ 405.00 or submit the accompanying Prisoner Financial Application along with a certified copy of his prison trust account statement for the six months immediately preceding the filing of his complaint.
