ORDER - IT IS THEREFORE ORDERED: 1. Plaintiffs Motion to Amend, ECF No. #15 , is DENIED without prejudice. 2. Plaintiff shall have until 30 days from the date of this Order, to file an amended complaint that clearly states a claim or claims upon which relief may be granted against any and all named defendants in accordance with this Order. If Plaintiff requires additional time to comply with this order, Plaintiff may request an extension before the deadline for compliance has passed. If Plaintiff fails to file an amended complaint by the deadline, the Court shall presume Plaintiff no longer wishes to amend his Complaint and shall proceed with performing an initial review of the Complaint and Supplement in accordance with 28 U.S.C. 1915(e) and 1915A(b). 3. If Plaintiff elects to file an amended complaint, it shall be filed as a single document as an amended complaint will supersede, not supplement, his previously complaint and supplement. Plaintiff must include all of the claims he wishes to pursue against all of the defendants he wishes to proceed against in the amended complaint, without relying upon or incorporating by reference any allegations made in the original complaint or supplement. Plaintiff should be mindful to explain in his amended complaint what each defendant did to him, when and where the defendant did it, and how the defendants actions harmed him. 4. The Clerks Office is DIRECTED to send to Plaintiff a copy of the civil complaint form. Plaintiff is instructed to attach additional pages to the complaint form as necessary. 5.
