White: : Granting Plaintiff's motion to reconsider the dismissal of this action for failure to file a Statement of Institutional Accounts #8 and reopening case. Plaintiff alleges he submitted the Statement of Institutional Accounts to the mail room at his facility, however, the Court finds it has not been received for filing in this case. The Court Clerk is directed to send Plaintiff another form for the Statement of Institutional Accounts. Plaintiff is directed to file with the Court within twenty (20) days the completed Statement of Institutional Accounts, along with a financial accounting statement for the 6-month period immediately preceding the filing of the complaint.
