ORDER by Judge Claire V Eagan that plaintiff's motion for leave to proceed in forma pauperis (Dkt. # 2) is granted. Within thirty (30) days of the entry of this Order, or by February 6, 2019, plaintiff shall submit to the Clerk of Court one completed summons, one completed U.S. Marshal Service form (form USM-285), and one copy of the complaint for service on each defendant named and identified in the complaint. The Clerk of Court shall send plaintiff nine blank summonses and nine blank USM-285 forms for plaintiff's use in complying with this Order. Plaintiff's address is set forth in the complaint (Dkt. # 1).
