ORDER by Chief Judge John E Dowdell The motion for leave to proceed in forma pauperis (Doc. 2) is denied. No later than 30 days after entry of this order, or on or before July 1, 2021, Washington shall either (1) pay in full the $350 filing fee and $52 administrative fee or (2) file an amended motion for leave to proceed in forma pauperis, on the court-approved form, and submit trust fund account statements for the six-month period immediately preceding the filing of his complaint. The Clerk of Court shall send to Washington one blank motion for leave to proceed in forma pauperis (form AO-240) marked "amended" and identified for Case No. 21-CV-0208-JED-SH. No later than 30 days after the entry of this order, or on or before July 1, 2021, Washington shall file a signed, amended complaint on the court-approved form.
