June 1, 2021 Opinion or Order Filing 10 ORDER by Chief Judge John E Dowdell The motion for leave to proceed in forma pauperis (Doc. 2) is denied. No later than 30 days after entry of this order, or on or before July 1, 2021, Washington shall either (1) pay in full the $350 filing fee and $52 administrative fee or (2) file an amended motion for leave to proceed in forma pauperis, on the court-approved form, and submit trust fund account statements for the six-month period immediately preceding the filing of his complaint. The Clerk of Court shall send to Washington one blank motion for leave to proceed in forma pauperis (form AO-240) marked "amended" and identified for Case No. 21-CV-0208-JED-SH. No later than 30 days after the entry of this order, or on or before July 1, 2021, Washington shall file a signed, amended complaint on the court-approved form. If the plaintiff is granted in forma pauperis status, he will not be required to pay the $52.00 administrative fee. Failure to comply with this order may result in dismissal of this action. Shreder: Upon review of the file, the court finds plaintiff has not submitted a proper civil rights complaint on a form approved for use in the Eastern District of Oklahoma. The plaintiff, therefore, is directed to file with the Court Clerk an amended complaint within 21 days or by 5/31/21.
