ORDER by Judge Claire V Eagan - The Clerk of Court shall send to Smith one blank 28 U.S.C. 2241 petition for writ of habeas corpus (form AO-242) and one blank civil rights complaint (form Pris-14), along with any instructions for completing those forms. No later than 30 days after the entry of this order, or on or before March 28, 2022, Smith shall submit whichever court-approved form or forms he deems appropriate for the action or actions he intends to pursue. The Clerk of Court shall send to Smith one blank motion for leave to proceed in forma pauperis (form AO-240). No later than 30 days after the entry of this order, or on or before March 28, 2022, Smith shall either (1) pay the filing fees necessary to commence whichever action he intends to pursue or (2) file a motion for leave to proceed in forma pauperis, on the court-approved form, requesting leave to proceed without prepayment., setting/resetting deadline(s)/hearing(s): ( Filing Fee due by 3/28/2022, Miscellaneous Deadline set for 3/28/2022), directing court clerk to take action (Re: #1 PETITION for Writ of Habeas Corpus - 2241 ) (RGG, Chambers)
