ORDER by Judge Claire V Eagan - Saylor's motion for leave to proceed in forma pauperis (Dkt. # 2) is denied. On or before December 9, 2022, Saylor shall either (1) pay in full the $350 filing fee and $52 administrative fee or (2) file an amended motion for leave to proceed in forma pauperis on the court-approved form and submit with the amended motion a statement of institutional accounts completed and signed by an appropriate jail official and a copy of his trust fund account statement for the six-month period immediately preceding the filing of the complaint. The Clerk of Court shall send to Saylor one blank motion for leave to proceed in forma pauperis (form AO-240) marked "amended" and identified for Case No. 22-CV-0481-CVE-SH. On or before December 9, 2022, Saylor may file an amended complaint, on the court-approved form, to cure the deficiencies identified in this order. The Clerk of Court shall mail to Saylor one blank 42 U.S.C. 1983 civil rights complaint (form PRIS-14), marked "amended" and identified for Case No. 22-CV-0481-CVE-SH, along with instructions for completing the form. ; setting/resetting deadline(s)/hearing(s): ( Filing Fee due by 12/9/2022, Miscellaneous Deadline set for 12/9/2022); directing court clerk to take action; denying #2 Motion for Leave to Proceed in Forma Pauperis (Re: #1 Complaint, #2 MOTION for Leave to Proceed in Forma Pauperis ) (RGG, Chambers)
