ORDER by Judge Claire V Eagan the Clerk of Court shall note on the record the substitution of Luke Pettigrew in place of Tom Bates as party respondent; the motion for leave to proceed in forma pauperis (Dkt. # 2) is denied; no later than 21 days after the date of this order, or on or before March 20, 2023, Allen shall either pay the $5 filing fee or file an amended motion for leave to proceed in forma pauperis, on the court-approved form, with a properly completed statement of institutional accounts signed by an appropriate prison official; the Clerk of Court shall send to Allen one blank motion for leave to proceed in forma pauperis (form OKND PRIS-ifp) marked amended and identified for Case No. 23- CV-0064-CVE-JFJ; the Clerk of Court shall send to Allen the unsigned signature page of the petition (Dkt. # 1, page 15), a postage prepaid pre-addressed envelope that Allen can use to return the signature page after he has signed it, and a copy of this order; Allen shall sign the signature page in the space designated for Signature of Petitioner and promptly return the signed signature page to the Clerk of Court; upon receipt of the signed signature page, the Clerk of Court shall replace the unsigned signature page with the signed signature page ; setting/resetting deadline(s)/hearing(s): ( Filing Fee due by 3/20/2023); directing court clerk to take action; Having reviewed the docket entry and/or PDF for Dkt #, the Court has determined that .
