ORDER by Judge Claire V Eagan - ORDER DIRECTING PLAINTIFF TO SUBMIT CORRECTED SERVICE DOCUMENTS - Plaintiff Henry Joseph Jaquez, a self-represented prisoner appearing in forma pauperis, brings this action under 42 U.S.C. 1983. Dkt. # 1. Mr. Jaquez's motion for issuance of summons (Dkt. # 6) is denied. The Clerk of Court shall send to Mr. Jaquez: (1) the summonses and service of process forms he submitted on September 30, 2025, so that he may use them to prepare new service documents; (2) seven blank summonses (form AO-440), identified for Case No. 25-CV-0522-CVE-JFJ; and (3) seven blank service of process forms (form USM-285), identified for Case No. 25-CV-0522-CVE-JFJ. ORDER by Judge Claire V Eagan - ORDER DIRECTING CUSTODIAL AGENCY TO COLLECT AND SUBMIT FILING FEE PAYMENTS ON BEHALF OF PRISONER-PLAINTIFF - This Court authorized Henry Joseph Jaquez (DLM # 1227365), a self-represented plaintiff detained at the David L. Moss Criminal Justice Center in Tulsa, Oklahoma, to proceed with this civil action without prepayment of the full filing fee. 28 U.S.C. 1915(a). The Court ordered Jaquez to submit an initial partial payment of $39.00 on or before November 7, 2025, and, thereafter, to make monthly payments until the $350 filing fee is paid in full. 28 U.S.C. 1915(b)(2). Beginning in December 2025, officials at the David L.
