ORDER -- Plaintiff appearing pro se, brings this action purportedly alleging violations of his civil rights. Currently before the Court is a document filed by Plaintiff and entered as a Complaint ECF No. #1 . A review of this document has been conducted pursuant to 28 U.S.C. 1915A(a). The Court now requires Mr. Whithead to file an amended complaint, utilizing the Court's approved form. ADDITIONALLY, in order for this action to proceed, Plaintiff must either pay the $405.00 filing fee or be granted leave to proceed without prepayment of fees. 28 U.S.C. 1914; 1915; LCvR3.2.
