Filing 4 ORDER - Plaintiff, representing herself, filed an application to proceed in forma pauperis. In her application, Plaintiff responded "yes" to the question of whether she receives disability payments. In the question for the amount received, she responded "100% service connected disabled veteran." In the question for the amount expected to receive in the future, she responded "100% service connected disabled veteran." These responses are insufficient. Plaintiff must provide the annual or monthly amount she has received from her veteran's disability payment in response to the first question and the annual or monthly amount she expects to receive in the future in response to the second question. Additionally, Plaintiff did not date and sign the form where required on page four to sign under penalty of perjury. Plaintiff must sign, either manually or electronically by typing s/ and typing her name (see Local Rule 11-1(a)), for her application to proceed in forma pauperis to be valid. The Court therefore DENIES Plaintiff's application to proceed in forma pauperis, ECF #2 , with leave to file by September 29, 2023, an amended application that cures the deficiencies identified in this Order. If Plaintiff does not timely file an amended application to proceed in forma pauperis, Plaintiff must pay the filing fee by October 6, 2023 to proceed with this case, or the Court will dismiss this case for failure to pay the filing fee.
