ORDER: The Court DENIES Plaintiffs Application to Proceed In Forma Pauperis (ECF No. #4 ) and orders Plaintiff to file an amended application curing this deficiency within thirty days of this Order. If Plaintiff is unable to obtain a certified copy of his prison trust account statement, he shall provide a detailed summary of his efforts to obtain a certified copy, including the names of the corrections officers involved and the reasons given for denying his request.
