MEMORANDUM OPINION & ORDER The Complaint filed in this action has been received without a filing fee or the forms required to proceed in forma pauperis in a civil rights case. This action may not proceed unless the Plaintiff either,1) tenders to the "Clerk, U.S. District Court" the statutory filing fee in the amount of $350.00, plus a $50.00 administrative fee, for a total of $400.00 or, 2) files a properly completed application to proceed in forma pauperis, including "an affidavit that includes a statement of all assets such prisoner possesses that the person is unable to pay such fees or give security therefor."
