October 5, 2018 Opinion or Order Filing 5 TEXT ORDER dismissing #4 MOTION for Leave to Proceed in forma pauperis by NICHOLAS JAMES KEREMES without prejudice. While Plaintiff has provided certain documentation on his own behalf in support of his Motion for Leave to Proceed in forma pauperis, he has failed to provide an Affidavit from the Records Officer at SCI Albion and a certified account statement for the six (6) months preceding the filing of the complaint as required by this Court. As stated in the Court's Order of 8/23/2018 at ECF No. #3 , Plaintiff may reopen the case by (i) tendering to the "Clerk, U.S. District Court," a statutory filing fee in the amount of $350.00, plus an administrative filing fee in the amount of $50.00, for a total of $400.00 or (ii) submitting another motion to proceed in forma pauperis along with the required affidavit and a certified copy of the inmate account statement for the six (6) months preceding the filing of the complaint. The case shall remain administratively closed until Plaintiff tenders such fee or submits a completed in forma pauperis application as required by 28 U.S.C. 1915(a)(1) and (2) and this Court's Orders.
