Filing 11 TEXT ORDER. While Plaintiff has provided certain required documentation the affidavit and a certified copy of the inmate account statement for the six (6) months preceding the filing of the complaint and the Authorization form authorizing the withdrawal of funds from an inmate account to satisfy the filing fee payment requirement pursuant to 28 U.S.C. 1915 (See ECF Nos. #4 , #5 , #9 , and #10 ) Plaintiff has failed to submit the Court's form motion to proceed in forma pauperis. Therefore, IT IS ORDERED that, on or before 08/14/2021, Plaintiff shall complete and file with the Clerk of Court the enclosed form motion to proceed in forma pauperis. Plaintiff does NOT need to resubmit an account statement or authorization form. The case shall remain administratively closed until Plaintiff complies with this Order.
