ORDER re #1 Motion for Leave to Proceed In Forma Pauperis by KAYLA CALLOWAY. On September 26, 2023, Ms. Calloway, who is pro se, filed a Motion for Leave to Proceed in Forma Pauperis utilizing an incorrect form, the In Forma Pauperis Affidavit Petition issued by the Unified Judicial System of Pennsylvania for Magisterial District Court. Ms. Calloway's Motion was also incomplete insofar as it did not provide a dollar amount in response to certain questions and the boxes at the end of the form are unchecked. Accordingly, IT IS HEREBY ORDERED that the #1 Motion for Leave to Proceed in Forma Pauperis is DENIED without prejudice. Ms. Calloway shall file a renewed motion for leave to proceed in forma pauperis or pay the applicable filing fee on or before 10/16/2023.
