The Court notes that the Motion at Docket No. #2 does not comply with 28 U.S.C. 1915 since it is not accompanied by the necessary affidavit nor the "certified copy of the trust fund account statement (or institutional equivalent) for the prisoner for the 6-month period immediately preceding the filing of the complaint or notice of appeal" required by 28 U.S.C. 1915 (a) (1) and (2). Furthermore, pursuant to 28 U.S.C. 1915 (b)(1), "if a prisoner brings a civil action or files an appeal in forma pauperis, the prisoner shall be required to pay the full amount of a filing fee."
