Filing 4 ORDER: Because Petitioner apparently seeks relief from more than one conviction and/or sentence, he must decide which conviction and/or sentence he wishes to challenge in this petition and file a new, separate petition in order to challenge the other conviction and/or sentence. Consequently, within 30 days of date he receives this Order, Petitioner is DIRECTED to submit a written statement indicating which conviction and/or sentence he wishes to challenge in this petition. Further, in order to file a civil lawsuit, a petitioner must submit the civil filing fee or an application to proceed in forma pauperis. If seeking pauper status, a petitioner must provide a certified copy of his inmate trust fund account statement for the 6-month period immediately preceding the filing of his petition as required by 28 U.S.C. 1915(a)(2). If the facility does not maintain inmate trust fund accounts, a petitioner may a submit a signed statement in lieu of a certified inmate account fund statement. Petitioner has submitted neither the filing fee nor an application to proceed in forma pauperis. The Clerk is DIRECTED to mail Petitioner a blank application to proceed in forma pauperis for prisoners. Petitioner, in turn, is DIRECTED to do one of the following within 30 days of the date he receives this Order: either return to the district court a completed application to proceed in forma pauperis and a certified copy of his inmate trust fund account statement for the 6-month period immediately preceding the date of the entry of this Order, as required by 28 U.S.C. 1915(a)(2); or submit the full civil filing fee of five dollars ($5.00) to the Clerk of Court.
