DEFICIENCY ORDER. It is ORDERED that the Clerk of Court forward a standard 1983 prisoner civil rights form to Plaintiff. It is further ORDERED that Plaintiff has thirty days from the receipt of this Order to complete the civil rights form and to submit it to the Court. Plaintiff should be careful to place the cause number, Civil Action No. 4:23cv1026, on the form and all other documents that he submits as part of the present case. It is finally ORDERED that Plaintiff has fourteen days from the receipt of this Order to either pay the $402.00 filing fee or submit an application to proceed in forma pauperis along with a certified data sheet reporting his trust fund activities for the last six months.
