ORDER. It is ORDERED the Clerk send a standard 1983 form and an application to proceed in forma pauperis to Plaintiff along with this Order. It is also ORDERED Plaintiff has fourteen days in which to complete the standard 1983 form clearly and legibly and submit it to the court if he wishes to pursue a civil rights lawsuit.
